Client is seeking an experienced Wound / Skin Care Nurse Coordinator to join our clinical team. Candidates must hold a current NYS RN license. We offer competitive compensation and full benefits in a supportive long-term care environment.
RESPONSIBILITIES:
Conduct weekly wound rounds with consultants and providers
Complete MD and family notifications; update care plans accordingly
Perform weekly skin monitoring and wound assessments in PCC
Review new admissions/readmissions for accurate wound documentation
Evaluate new or worsening wounds promptly
Review and enforce pressure ulcer prevention measures
Recommend and coordinate pressure-relief devices
Update individualized care plans
Review treatment orders and interventions for accuracy
Complete wound-related audits
Collaborate with interdisciplinary teams: dietitian, rehab, social work, nursing
NEW ADMISSIONS:
Verify completion of admission skin assessments within 24 hours
Add appropriate residents to the wound care provider list
Note: The ADON oversees the facility’s overall wound care program, including the wound tracker and monitoring of new admissions and new wounds.
QUALIFICATIONS:
Nursing degree or graduate of an approved nursing program
Current NYS RN license required
Wound Care Certification preferred
Long-term care experience strongly preferred
Previous wound care experience in a healthcare setting preferred
